Informed Consent to Education and Exercise (Required Documentation)
 

Each participant in any of the Gymflesh® programs must submit a completed and signed "Consent to Education and Exercise Form" for our review and apprival. Download the form here.  Please fill out the form, sign it, and mail the original to: Gymflesh® Gym, POB 475, Tappan, NY 10983-0475.

Some participants, at our professional discretion, may be asked to submit a supplemental "Confidential Health Status Form" for our review and approval. Failure to accept in part or in whole any terms and conditions of these "Participant Informed Consent Forms" will not allow you or your designee to participate by any manner whatsoever in any Gymflesh® program, nor does a submission of these application forms assure acceptance into Gymflesh Corporation or any engagement or commitment of Gymflesh® services.

 

Gymflesh® Terms of Service, including prerequisites and conditions, depend upon specific requests. Service Agreements, whether individual or group, define, but are not limited to, the following items:

4.   Interruptions of emergency

5.   Postponement

6.   Cancellation

7.   Bereavement

8.   Number of participants

9.   Program fees

10. Facility requirements

11. Nutrition

12. Program eligibility

13. Representations by the host/s

14. Confidentiality

15. Proprietary nature of the programs

16. Tax exempt status of a client (if applicable)

17. Additional agreement terms as necessary

All payments, payment terms and receipts must be received and certified by the Gymflesh® Bursar at 212-714-8000, ext. 5, and at bursar@gymflesh.com.  Complete payment is due in full upon receipt of any Service Agreement.

 

Final approval, acceptance, and matriculation into any Gymflesh Corporation program must be issued in writing by the Gymflesh® School and University Project Provost or by the Provost's assignee at 212-714-8000, ext. 3, and provost.school@gymflesh.com.

Consultation Terms of Service, Fees, and Legal Notices
 

1. Individual requests start at USD $700.00 per 90-minute session (15 hours minimum, 10 sessions), exclusive of customized rates and additional fees.*

 

2. Corporate and group reservations start at USD $5,000 per day (one 6-hour appointment minimum), exclusive of customized rates and additional fees. Billing-hour services (pay by the hour offerings) are not available for corporate or group requests.*

 

* Particular Terms of Service Agreements, conditions, and additional rates and fees depend upon specific requests and particular needs.
 

N.B. Gymflesh® offers significant discounts for qualifying Protected Classes victims and survivors of discrimination, harassment, hate crimes, and other human rights violations (all communications are restricted and confidential). Below are two samples of predesigned Gymflesh® School and University Project "Limited Edition" and "One-Day Comprehensive Edition" Fire Service Protocol programs. These paradigms are format examples similar to other specific areas of group concentration (military, police, et al.) and are not representative of all programs models which vary by frequency, intensity, duration, student enrollment, certifications, mandates and standards, and the like.

 

Gymflesh® School and University Project "Build-Your-Own Edition" programs are designed according to the host's particular requirements, and fees are adjusted accordingly.
